GENERAL INFORMATION

Scheduling
We attempt to provide each family with the most efficient and informative appointments. We strive to see all patients on time for their scheduled appointments. Please remember, we run on children’s time, not adult time. There are times when our schedule is delayed in order to accommodate an injured child, apprehensive or scared child. Please accept our apology in advance should this occur during your appointment. We will provide the exact same courtesy if your child is need of emergency treatment.

Emergencies
Our treatment goal is to avoid the unexpected. However, if an emergency of concern arises, don’t hesitate to call us. Questions can be immediately answered Monday-Friday. If the emergency should occur after office hours, call our office and the answering machine will supply our emergency phone number. A pediatric dentist should always be on call. Dr. Mendoza or Dr. Vergotine are available by pager and Dr. Eltink is also available to answer over the phone any orthodontic emergency that may occur or concern you have.

Appointments/ Running Late
Your scheduled appointment time has been reserved specifically for your child. While we make every effort to remind you with a courtesy phone call of your appointment your appointment is your responsibility. If a cancellation is unavoidable, please call the office at least 24 hours in advance, this allows us sufficient time to notify another patient needing our care.  If you arrive 10 minutes late for your appointment, you may be asked to reschedule for the next available appointment time.

Broken/ Rescheduled/ Failed Appointments
Any broken, last minute rescheduled or failed appointment effect many people. If you do not call to cancel and fail to show as scheduled to your child’s appointment, you may be charged a broken appointment fee of $55.00.  If you wish to reschedule your child’s appointment, we ask that you give us at least 24 hours advance notice. Cancellations received less than 24 hours will be considered a last minute failed appointment.

If two (2) broken/failed appointments occur or two (2) cancellations without a 24 hour notice, we may request that you seek dental care at another dental office that can better accommodate your schedule. Our office reserves the right NOT to schedule any subsequent appointments. All broken appointment fees must be paid before we are able to reschedule your child’s appointment.
*Families who fail, brake or reschedule last minute appointments with multiple children scheduled may not be allowed to schedule their children all on the same day again.

Insurance
Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary greatly, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Please be aware that your insurance policy is simply an agreement between you and your insurance company, and is not a guarantee of payment. Therefore it is your responsibility for what your insurance does not cover. Your estimate patient portion must be paid in full at time of service. In you have any questions our courteous staff is always available to answer them.

*Important note about insurance coverage: insurance providers we work with may change at any time without notice.

*Most importantly, please keep us informed of any insurance changes such a policy name, insurance company address, or change of employment.

Co-payment/Payments Accepted
Your co-payments have to be paid up front before the services are rendered. Our office accepts payments with Visa, American Express, Discover, Master Card, Debit Cards that bear the Visa or Master Card logo and also Cash.

Public Aid /Medical Card/ Illinois Medicaid/All Kids
Our Office accepts all of the above. Please have current card showing eligibility prior to your visit. If for some reason you do not have your card before your visit, please call our office so we can reschedule your appointment. Often there is lapse in coverage for which the patient will be responsible for all fees incurred during that time. Payment for all non-covered services by insurance will be the responsibility of the patient.

Preventive Dentistry: Routine cleaning, topical applications of fluoride, fluoride varnishes and rinses. Oral hygiene counseling, diet recommendations, dental sealants, etc.

Interceptive and Comprehensive Orthodontic Treatment by orthodontist specialist Dr. Anthony P. Eltink.

Restorative Dentistry: Silver fillings, tooth colored fillings, colored bonding restorations, porcelain and stainless steel primary teeth crowns, space maintainers, other retainers, etc.

Dental Management of Behaviors: Psychological approaches; nitrous oxide “laughing gas”; pre-medication; conscious sedations; IV sedations and general anesthesia. In occasions due to high dental need, young age and/ or special dental needs,  the best approach for the delivery of dental care will be General Anesthesia, and these services are offered by Dr. Mendoza at Advocate Illinois Masonic Medical Center, where he has been teaching for the last 9 years. Please feel free to inquire about this modality.
